
Community Health Worker Training Program
In accordance with our mission and our vision, Heritage Care, Inc. provides a variety of Maryland-certified healthcare and public health training programs. Our goal is to help improve health outcomes as well as social determinants of health for underserved members of our community.
Heritage Care Inc, is certified by the state of Maryland Department of Health to offer a community health worker (CHW) program. Our organization works with local workforce development organizations and colleges to provide community members with this training free of cost* Heritage Care also gives income-based scholarships to community members that elect to pay out of pocket.

Role of a Community Health Worker
CHWs are widely accepted as key stakeholders in our healthcare system and have a major impact on public health. A CHW is a public health professional that works with clients from underserved communities. CHWs work in many different capacities and provide a wide variety of services to the community. Some of these services include:
Informal Healthcare Counseling
Assistance Accessing Local Healthcare Resources
Outreach and Advocacy
Community Capacity Building
Areas of Specialization
CHWs can also specialize in different areas. Some of those areas include:
Geriatrics
Supporting Victims of Trauma
Mental Health
Promoting the Health of Formerly Incarcerated People
Diabetes Prevention
Sexual and Reproductive Health

Classroom Instruction and Practicum Experience
Training Structure
The Heritage Care Inc. CHW training program includes 155 hours of classroom instruction and 40 hours of practicum instruction. Classes are offered in person and our schedule for classes is flexible. CHW Apprenticeship
Program in Diabetes
Heritage Care is proud to announce that we have partnered with Totally Linking Care (TLC) to offer a Maryland Department of Labor-certified CHW apprenticeship program that specializes in chronic disease management and diabetes prevention. Apprentices will complete Heritage Care's CHW program along with two supplemental courses- CDC Prevent T2 Lifestyle Coach Training and Diabetes Community Care Coordinator certification. Apprentices will be employed full-time by TLC during their apprenticeship. Apprentices will be paid while they study the required certifications. Apprentices will receive an increase in pay as they complete their certifications and gain field experience. The duration of this program is one year.
